“What’s on Your Mind?”

The Institute for Nuclear Power Operations Anatomy of an Event illustrates the life cycle of error, originating from a latent organizational weakness through the event itself. Included in this life cycle are flawed defenses, error precursors and an individual’s initiating action. Too often after the event, organizations view the actions and decisions of this individual through the lens of backward accountability, reinforcing a mistaken notion of a single point of failure. Predisposed to search for the malefactor, contaminated by a cultural history of viewing accidents through a ‘gotcha’ mindset, corrective actions become a cycle of misdiagnoses and faulty prescriptions. This interactive workshop will introduce participants to common mental biases associated with event-triggering actions and provide specific techniques and countermeasures for improving personal error detection and recovery.

Safety Fest TN

Safety Fest TN will be held September 12 – 16 at multiple locations in Oak Ridge and Knoxville, TN. The class schedule and registration have launched! The event, which started in 2012, provides free training to promote safety and health at work, at home and in the community. And what a community it is! Last year’s event had over 850 people registered from 79 cities in 6 states, representing 172 organizations. It included 66 free safety classes, seminars and demonstrations. The 51 Instructors were graciously provided by organizations giving back to the community. This year’s event is going to be bigger and better. Your organization can partner, sponsor or provide resources to promote safety and health too!
